Paradoxic action of amyl nitrite in coronary patients.

چکیده
coronary graphic changes typical of temporary coronary insufficiency. It is suggested that, in such cases, a slight increase of cardiac work (tachycardia) together with a decrease of blood supply (fall in blood pressure), leads to myocardial ischemia. Comparison between an exercise test and amyl nitrite inhalation in the same patients indicates similar results. Amyl nitrite inhalation may be used as

Paradoxical electrocardiographic effects of amyl nitrite in coronary artery disease.
i had angina with normal coronary arteries. This group was compared with i5 patients who had angiographically proven coronary artery disease and positive treadmill tests but negative amyl nitrite tests: the group with positive amyl nitrite tests responded to drug inhalation with a significantly greater heart rate increase (47% vs. 34%, P< o.oI), and a significantly greater fall in systolic bloo...
متن کاملActions of the nitrites on the peripheral circulation and myocardial oxygen consumption: significance in the relief of angina pectoris.
Myocardial ischemia is best viewed as a discrepancy between myocardial oxygen demands and availability of oxygen to the heart. Although the fundamental action of each nitrite is generalized direct v d l a t i o n , in patients with coronary arterial disease the dynamic factor mediating favorable effects of these agents on angina pectoris appears largely to be reduction of myocardial oxygen requ...

coronary arteries, -4 or that they diminish the oxygen requirements of the myocardium by diminishing the external work of the heart.5'6 Much of the confusion concerning the actions of these drugs has arisen from the assumption that nitroglycerin and amyl nitrite have similar effects. Moreover, many studies on the peripheral circulatory effects of nitrites have been carried out with sodium nitri...
متن کاملEffect of myocardial shortening velocity on duration of electrical and mechanical systole. S2T interval as measure of shortening rate.
To determine the effect of myocardial shortening velocity on the duration of electrical and mechanical systole, five healthy men, ages 32 to 41, were studied. Carotid message and atropine were used to define the effects of changes in heart rate without changes in shortening rate. The effects of amyl nitrite, which produced approximately the same degree of tachycardia as atropine but a significa...
متن کاملProfound bradycardia after amyl nitrite in patients with a tendency to vasovagal episodes.
Two patients with mild aortic insufficiency inhaled amyl nitrite during routine echocardiographic examinations. One developed sinus arrest and syncope and the other had pronounced sinus bradycardia. The mechanism of this paradoxical response is unclear. Caution should be exercised when amyl nitrite is administered for diagnostic purposes.
